CARE's outpatient rehabilitation programs are designed for people who have lost their functional autonomy, either after hospitalization or as part of a stabilized chronic pathology. How does the treatment work?
On medical prescription, an interdisciplinary assessment is carried out at the start of the program. This comprehensive assessment identifies your abilities, limitations and expectations.
The therapeutic journey is then co-constructed with you, according to a personalized schedule including :
- Physiotherapy sessions
- Occupational therapy
- Educational workshops
Program duration and frequency are adjusted according to your progress.
